Tips for Keeping Your Schedule Full

One of the greatest sources of frustration in any dental office is when a perfectly planned schedule falls apart. Your team may have followed every scheduling guideline, only to receive several same-day or next-day cancellation calls. Suddenly, what looked like a productive day turns into a scramble to fill openings, leaving everyone frustrated.

While you can’t control patient behaviour, you can implement systems that encourage patients to keep their appointments and reduce the number of cancellations.

Have Hygienists Schedule Their Own Patients

Patients who schedule their next appointment with their hygienist before leaving the treatment room are often more likely to keep that appointment. Many patients develop a strong relationship with their hygienist and are less inclined to disappoint someone they know and trust.

Make Appointments More Personal

Keep personal notes from previous visits in the patient’s chart. Patients appreciate it when you remember details from past conversations, whether it was a vacation they were planning or the raccoons they were trying to remove from their attic. These small details help patients feel valued rather than just another appointment on the schedule.

Classify Your Patients

One effective way to manage your schedule is to classify patients as either high-risk or low-risk.

High-risk patients typically have unpredictable schedules, frequently cancel or reschedule appointments, may have a poor payment history, and are less likely to follow treatment recommendations. These patients should generally not be pre-appointed. Instead, book them one week or less in advance, when they are more likely to keep their commitment.

Low-risk patients have stable schedules, rarely change appointments, maintain a positive payment history, and consistently follow treatment recommendations. These are the patients who should be pre-appointed.

Be sure to use a classification system within your practice management software that cannot be easily interpreted by patients who may see the screen.

Review Your Cancellation Process

Take a close look at how cancellation requests are handled in your office. Whenever possible, avoid accepting cancellations by text message or voicemail. It is much easier for patients to cancel when there is no conversation involved.

When a patient calls to cancel, your administrative team should be prepared with language that reinforces the value of the appointment or helps overcome common scheduling obstacles. Often, a simple conversation can save an appointment that might otherwise have been lost.

Use the 2-2-2 Confirmation Rule

For patients who have pre-booked appointments, consider using the 2-2-2 confirmation approach:

  • Confirm the appointment two weeks in advance.
  • Send another reminder two days before the appointment.
  • Ask if they would like a final reminder two hours before their visit.

Regular communication helps reduce forgotten appointments and reinforces the patient’s commitment.

Be Thoughtful with Cancellation Fees

Many practices have a policy that charges patients for same-day cancellations. While this policy can encourage patients to keep their appointments, it should be used carefully. Charging the fee may damage the patient relationship and could even result in losing the patient.

Instead, the policy is often most effective as a reminder.

For example:

“We do have a $75 same-day cancellation fee, and we’d hate to apply that to your account. Is there any way we can help you keep today’s appointment?”

This approach encourages patients to reconsider while maintaining a positive tone.

Your schedule is one of the most valuable assets in your practice. A well-managed schedule improves productivity, reduces stress, and creates a better experience for both your team and your patients. When the day runs smoothly, everyone leaves the office feeling more accomplished and better prepared for tomorrow.
